Jennifer Grant, Quality Specialist
Jennifer Grant, MS PT, has worked at MedStar VNA for more than 15 years. She started working with the organization as an in-home physical therapist, providing functional training and educating patients and families on home safety, therapeutic exercises, and helping them achieve independence in the home. She built on this background and knowledge and became a care manager, providing support and education to clinicians, care oversite, and service recovery.
Since then, Jenn has advanced to the role of quality specialist. In this role, she assists in maintaining Joint Commission readiness, participates in regulatory matters, identifies best practices for patient care delivery, and spearheads important clinical program committees like Fall Prevention and Prudent Care. In fall of 2018, she received an award from the Maryland-National Capital Homecare Association for her extensive work on Fall Prevention. She is an active member of the American Physical Therapy Association and their committees on geriatric and homecare.
Jenn loves collaborating with her tight-knit quality team. “Everybody on the team has amazing strengths that complement one another,” she said. “We are very supportive of one another and we work together to enhance patient safety and care. The most rewarding part of the job for me is when I see improved patient outcomes due to our team’s efforts,” Jenn elaborated.
Currently, her team is working to share their collective knowledge regarding Fall Prevention in the Home with other MedStar Health entities, to improve care and outcomes for an even larger population of patients across the care continuum.
Jenn and other members of the Quality Department collect and analyze data from occurrence reports related to patient falls, medication events, infections, and other patient outcomes data to improve MedStar VNA’s processes, develop clinical and patient education materials, and advance patient safety.
Jenn also makes a positive difference through her leadership role with the Prudent Care Committee. This group, which includes MedStar VNA’s medical director, discusses difficult patient cases and identifies resources and strategies which help nurses, therapists, and social workers better support patients who experience serious challenges at home or with their healthcare conditions.
Jenn plans to continue honing her skills related to healthcare quality assurance, specifically related to assessing and communicating quality metrics aimed at identifying improvement approaches and enhancing the quality of MVNA’s patient care.
